Beyond the Monofilament: Diagnostic Tools That Enhance Neuropathy Detection

The 10g monofilament test has long been the standard screening tool for peripheral neuropathy. But by the time a patient fails that test, the nerve damage is often significant and advanced.

As a private or direct-pay physician, you can do better. You have the opportunity to detect neuropathy earlier, provide a more comprehensive evaluation, and develop care plans that actually improve outcomes.



The Limits of Traditional Screening

Monofilament testing is valuable, but:

  • It only assesses large fiber function

  • It doesn’t catch small fiber neuropathy, which presents earlier

  • It tells you nothing about temperature sensitivity, vibration loss, or early proprioceptive decline

In short, it’s a good start but not enough.



A Smarter, More Complete Evaluation

You don’t need an EMG lab to enhance your neuropathy diagnostics. With a few additional tools and five extra minutes, you can dramatically improve detection and patient understanding.

Consider Adding:

  • 128 Hz tuning fork to assess vibration loss

  • Cold/heat discrimination tools for small fiber evaluation

  • Pinprick or light touch testing for sensory distribution

  • Proprioception and Romberg testing for balance assessment

  • Reflex and strength testing to identify asymmetry or motor loss

These tools are simple, affordable, and well-suited for in-office use.



Patients Notice and Appreciate Thoroughness

When you go beyond the standard, patients feel seen. Especially those who’ve been told “everything looks normal” despite persistent symptoms. A detailed exam shows you're not just checking a box you’re looking for answers.

It also helps track improvement over time, giving patients a sense of measurable progress.
